Anthem Blue Cross (Anthem) offers a few different types of Medicare Advantage plans in California. One type of Medicare Advantage plan is a Special Needs Plan (SNP). Depending on where you live, you may be able to find an Anthem Medicare Advantage SNP in your area.

A Special Needs Plan is similar to other types of Medicare Advantage plans, but with a unique twist. The benefits included in these plans are custom-built to fit the specific needs of people who live with certain health conditions or those who have low incomes.
For example, a Chronic Conditions Special Needs Plan (C-SNP) for people living with diabetes mellitus may include more robust coverage of insulin, blood sugar monitors and other items or services that are commonly used to manage diabetes.
A Dual-Eligible Special Needs Plan (D-SNP) can give extra coverage to people who have both Medicare and Medicaid.
Institutional Special Needs Plans (I-SNPs) are designed for beneficiaries living in an Anthem network nursing home or assisted living community or other approved location.
Special Needs Plans offer the same basic benefits covered by Original Medicare (Medicare Part A and Part B). They may also offer additional valuable benefits such as:
A Special Needs Plan will also typically include dental, vision, hearing and prescription drug coverage.
Depending on where you live in California, you may be able to find an Anthem Medicare Advantage C-SNP that is designed to cover the following conditions.
An Anthem Medicare C-SNP for people who live with diabetes can include coverage for blood glucose monitors, insulin and other tools used to help manage blood glucose. Members also receive coverage for foot care as well as nutritional training and additional screenings and monitoring.
A C-SNP for people living with End-Stage Renal Disease (ESRD) covers nutritional training, prescription drugs and common treatments of the disease. The plans also offer care teams who work with your nephrologist and primary care physician to offer personal support.
These C-SNPs offer coverage for oxygen supplies and other related equipment used to help treat lung-related disorders such as chronic obstructive pulmonary disease (COPD), asthma, chronic bronchitis, pulmonary hypertension and emphysema. Network participants for these plans include pulmonologists (lung specialists) who work with your primary care doctor to help manage your care.
This type of C-SNP covers the necessary treatment you receive from cardiologists and your doctor to treat chronic venous thromboembolic disorder, chronic heart failure, cardiac arrhythmias, coronary artery disease or peripheral vascular disease.
D-SNPs are designed for beneficiaries who are dual-eligible, meaning someone who is eligible for both Medicare and Medicaid.
These plans include all Medicare Part A and B coverage, Medicaid benefits and prescription drug coverage combined into one plan. Anthem D-SNPs typically include some extra benefits such as money for over-the-counter health items and transportation services or in-home support.
